Student Complaint Forms must document that you followed the appropriate chain of command prior to submitting your request: instructor » department chair. This form is intended for use by students who are currently enrolled in CIS or QMST courses or are CIS majors. Complaints must involve a CIS or QMST course or faculty member teaching one of our courses. If your complaint does not concern the Information Systems and Analytics department or our faculty, please contact the appropriate department.
